A buyer's guide to surgical instruments
Two forceps can look identical and differ by fifteen years of service life. The differences are visible if you know where to look.

Surgical instruments are among the few purchases where a good buy genuinely lasts decades and a poor one becomes unusable within a couple of years. The price difference at purchase rarely reflects that gap.
Steel grade: what to ask
Surgical stainless steels fall into two broad families, and the right one depends on the instrument:
- Martensitic (410, 420, 440): Hardenable, holds an edge. Used for scissors, needle holders, chisels, osteotomes, rongeurs. Slightly less corrosion-resistant, which is the trade for hardness.
- Austenitic (304, 316): More corrosion-resistant, not hardenable. Used for retractors, trays, bowls, kidney dishes, cannulae.
An instrument sold simply as “stainless steel” with no grade stated tells you the seller either does not know or would rather not say.
Finish
Mirror finish is highly polished — easy to clean, easy to inspect, but reflects operating-theatre lighting back at the surgeon. Satin or matte finish reduces glare and is generally preferred for instruments in active use under strong light. Black or ebonised finishes exist for laser and electrosurgical use.
Neither mirror nor satin indicates quality by itself. What matters is that the finish is even, with no pitting, no burrs and no discoloured patches — those indicate poor polishing or contaminated steel.
Inspecting an instrument at the counter
Pick it up. Spend thirty seconds on it:
- Action: Open and close it repeatedly. Smooth throughout, no grinding, no catching.
- Box joints: No lateral play. Wobble at the joint means the instrument will loosen quickly.
- Scissor blades: Should meet along their full length. Hold to the light — no gap. Test on gauze at the tip, not just mid-blade.
- Serrations and teeth: Must align exactly when closed. Misaligned serrations mean tissue slips.
- Ratchets: Engage crisply at each position and hold. A ratchet that springs open under light pressure is a defect, not a characteristic.
- Tips: Meet precisely. On fine instruments this is where poor manufacturing shows first.
- Marking: Manufacturer name and grade should be laser-marked or stamped cleanly, not stickered.
Buying sets vs individual instruments
Pre-assembled procedure sets are convenient and usually cheaper per piece. The risk is paying for instruments your practice never uses while being short of the two you use constantly.
Our suggestion for a new practice: buy a core set, then add individual instruments once actual usage is known. For an established practice, build the list from what you actually reach for.
Care determines lifespan more than price
Most instrument failure we see is care-related, not manufacturing-related:
- Clean immediately after use. Dried blood and saline pit the passive surface layer. Saline is particularly aggressive — never leave instruments soaking in it.
- Use a neutral pH enzymatic cleaner. Strongly acidic or alkaline agents damage the finish.
- Open all joints and ratchets during cleaning and sterilisation, so surfaces are actually reached.
- Rinse with demineralised water. Hard water leaves deposits that look like rust and cause staining.
- Dry completely before storage. Moisture in a closed tray is the single most common cause of spotting.
- Lubricate box joints periodically with instrument milk.
- Do not mix metals in the same tray — contact between dissimilar metals causes electrolytic corrosion.
- Store sterile packs dated and rotated, protected from moisture.
If instruments do stain
Brown or blue-black patches are usually deposits rather than corrosion of the steel itself, and can often be addressed with an appropriate instrument stain remover. Actual pitting cannot be reversed — instruments with pitted surfaces cannot be reliably sterilised and should be withdrawn.

- Steel grade for each instrument type
- Finish — satin or mirror
- Manufacturer and country of origin
- Whether instruments are supplied individually inspected
- Availability of replacements for individual pieces from a set
- Warranty against manufacturing defect

Martensitic grades such as 410, 420 and 440 are used where hardness and edge retention matter — scissors, needle holders, osteotomes. Austenitic grades such as 304 and 316 are more corrosion-resistant and used for retractors, trays and bowls. Ask which grade applies to the specific instrument.
A mirror finish is highly polished and easy to clean but reflects light, which can be distracting under an operating lamp. A satin (matte) finish reduces glare and is generally preferred for instruments used under strong lighting. Both can be high quality.
Open and close it several times — the action should be smooth with no grinding. Check that box joints have no play, that scissor blades meet along their full length, that serrations align exactly, and that there are no pits, burrs or discoloured patches in the finish.
Usually staining rather than true rust, caused by residues, hard water deposits or being left wet after sterilisation. Rinse with demineralised water, dry fully before storage, and avoid leaving instruments soaking in saline or blood, which pits the passive layer.
