Pulse oximeters and thermometers for home use
Both devices are cheap and both are routinely misread. Cold hands alone can drop an oximeter reading enough to cause real alarm.

These are the two cheapest devices in a home-care setup and the two most likely to cause unnecessary panic, because both are easy to misread.
Pulse oximeters
A pulse oximeter clips onto a fingertip and shines light through it to estimate the oxygen saturation of the blood, plus the pulse rate. It is genuinely useful for anyone with a respiratory condition — it gives a clinician a concrete number over the phone.
What distorts a reading
Before treating a low number as real, check these:
- Cold hands — the most common cause by far. Poor peripheral circulation weakens the signal. Warm the hand and re-measure.
- Nail polish or artificial nails — dark colours block the light. Remove polish from one finger.
- Movement — keep the hand still and rested
- Poor positioning — the probe should sit squarely on the fingertip, not half on
- Bright ambient light — shield the probe from direct sunlight or strong lamps
- Very low blood pressure or weak pulse — the device needs a pulse signal to work at all
Reading it properly
- Warm the hand if it is cold
- Rest the hand at chest level, still
- Clip the probe on and wait for the numbers to settle — usually 30 to 60 seconds
- Check the pulse rate shown matches roughly what you feel at the wrist. If it does not, the reading is unreliable.
- Record the number, the time and what the patient was doing
Ask the treating clinician what range applies to your patient and at what reading you should call. Targets differ, particularly for people with chronic lung disease, where a lower baseline may be expected. Write the number down and keep it with the device.
What to look for when buying
- Clear, well-lit display readable at arm’s length
- A plethysmograph or signal-strength indicator, so you can see whether the reading is trustworthy
- Adjustable display orientation
- Auto shut-off to preserve batteries
- Paediatric probes if the device will be used for a child — adult probes do not fit small fingers and will read badly
Thermometers
Digital contact
Inexpensive, accurate when used correctly, and the most reliable option for home use. Slower than infrared — typically 30 to 60 seconds. Can be used orally, axillary or rectally depending on the model and the patient; follow one method consistently.
Infrared forehead (non-contact)
Fast and hygienic, and it can be used on a sleeping patient without waking them — a real advantage in home care. More sensitive to technique, distance and ambient temperature. Let the device acclimatise to room temperature before use.
Infrared ear (tympanic)
Fast and generally reliable in adults and older children. Technique matters — the probe must be positioned correctly in the canal — and earwax affects readings.
Practical points
- Be consistent. Different methods give different readings. Comparing a forehead reading today against an oral reading yesterday tells you nothing.
- Record the method alongside the number when writing observations for a clinician.
- Wait after hot or cold drinks before taking an oral reading.
- Keep a spare. Thermometers fail at inconvenient times and they cost very little.
Keeping a record
A simple notebook with date, time, reading and method is worth more at the next consultation than any single measurement. Patterns are what clinicians act on.

Before assuming the reading is real, check the obvious causes: cold hands, nail polish or artificial nails, poor positioning, movement, or a weak pulse signal. Warm the hand, remove polish, rest the hand at chest level and wait for the reading to stabilise. If a genuinely low reading persists, contact the treating clinician.
A digital contact thermometer used correctly is reliable and inexpensive. Infrared forehead and ear thermometers are faster and better for children or sleeping patients, but are more sensitive to technique and ambient conditions. Whichever you use, be consistent — comparing readings across methods is misleading.
Yes. Dark polish and artificial nails can block the light the sensor relies on and produce falsely low readings. Remove polish from at least one finger, or clip the probe to a toe or earlobe if the device supports it.
Target ranges are patient-specific and are set by the treating clinician, particularly for people with chronic lung disease. Ask what range applies to your patient and at what number you should call — then write it down and keep it with the device.
