Keeping medical equipment working: a practical maintenance guide
Most equipment we are asked to repair did not fail. It was allowed to fail — by a filter nobody cleaned or a seal nobody checked.

Almost all the equipment brought to us as “failed” had not reached the end of its life. It had a clogged filter, a perished seal, a worn castor or a battery nobody had tested — small things, left long enough to damage something expensive.
Here is a maintenance frame you can actually run.
The principle
Every piece of equipment needs three tiers of attention: a daily user check that takes seconds, periodic user maintenance that takes minutes, and professional service at the manufacturer’s interval. Skip the first two and the third becomes a repair.
Hospital and electric beds
- Daily: test each motor function through its full range; check castor brakes hold; check the hand control cable is not pinched under the frame
- Monthly: inspect the mains cable for damage; check side-rail latches engage firmly; clean castors of hair and thread, which is the usual cause of stiff wheels; check frame welds and fasteners
- Annually: professional check of motors, actuators and any battery backup
The commonest avoidable failure is a hand-control cable crushed by a castor. Route it and check it.
Oxygen concentrators
- Daily: check the flow setting has not drifted; confirm the humidifier bottle is clean and correctly filled; listen for a change in the compressor note
- Weekly: rinse and fully dry the gross particle filter; check tubing for kinks; check cannula condition
- Monthly: replace or clean the cabinet filter per the manual; confirm 30 cm clearance around air intakes
- Annually: professional service including sieve-bed assessment and oxygen concentration verification

Autoclaves
- Daily: check door seal for cracks and debris; confirm water reservoir level and quality; run the manufacturer’s test cycle where specified
- Weekly: clean the chamber and drain filter; check the safety valve per the manual
- Per protocol: chemical and biological indicator testing; retain cycle records
- Annually: professional service, pressure-vessel inspection and validation
Never run an autoclave on tap water where distilled or demineralised is specified. Scale destroys chambers and is not covered by any warranty.
Patient monitors and diagnostic devices
- Daily: confirm alarms are audible and limits are set for the current patient; check probe and cable condition
- Monthly: test battery runtime under load — a battery that holds 10 minutes is a battery that has failed; clean per protocol; inspect cuffs for leaks
- Annually: professional calibration and electrical safety testing
Wheelchairs, walkers and mobility aids
- Weekly: test wheel locks; check tyre condition and pressure on pneumatic tyres; check rubber tips on walkers and sticks
- Monthly: check frame fasteners, folding mechanisms, footrest latches and armrest locks
- As needed: replace worn tips and tyres immediately — these are the cheapest parts and the ones whose failure causes falls
Air mattresses
- Daily: confirm the pump is cycling; check pressure setting against patient weight; check tubing is unkinked
- Weekly: clean the pump air intake; inspect cells for leaks
- Ongoing: keep the pump where air circulates and off the floor
Keeping records that hold up
For institutions this is not optional. Maintain, per device:
- Brand, model, serial number and purchase date
- Warranty expiry
- Service interval and date of last service
- Fault and repair history
- Calibration or validation certificates where applicable
A simple spreadsheet is enough. What matters is that it exists, is current, and can be produced when asked.
Three habits that prevent most failures
- Clean the filters. On anything with a compressor or fan, this is the highest-value maintenance action available.
- Test batteries under load, not by looking at a charge indicator.
- Fix small faults immediately. A stiff castor, a frayed cable, a sticky latch — each is cheap now and expensive in six months.

Follow the manufacturer's stated interval for each device — it varies widely. As a general frame: daily user checks, monthly user maintenance such as filter cleaning, and annual professional service. Equipment in continuous use needs more frequent attention than the schedule assumes.
Cleaning and replacing external filters, checking cables and plugs for damage, inspecting castors and brakes, testing alarms, checking battery runtime, and cleaning per protocol. Anything requiring the case to be opened, or any calibration, should go to a qualified technician.
Usually because deferred maintenance accumulates. Filters left uncleaned make compressors work harder; worn seals stress motors. Equipment maintained on schedule commonly runs well beyond warranty; equipment that is not, fails around the point the warranty stops covering it.
