Air mattresses and preventing bed sores
A pressure sore can develop in hours and take months to heal. For a bedridden patient, the mattress is not a comfort item — it is clinical equipment.

If a family member is bedridden, pressure sores are the complication most likely to turn a manageable situation into a serious medical problem. They form where body weight presses skin against bone for too long — the sacrum, heels, hips, shoulder blades. Blood supply is cut off, tissue dies, and what began as a red patch becomes an open wound that can take months to heal and may need surgical care.
The encouraging part: they are largely preventable, and the equipment involved is neither exotic nor expensive.
How an alternating pressure mattress works
An air mattress system is a mattress made of inflatable air cells connected to an electric pump. The pump inflates one set of cells while deflating the other, then reverses, on a cycle of roughly 10 to 15 minutes.
The effect is that no single patch of skin carries continuous pressure. Blood supply to any given area is restored regularly, which is precisely the mechanism that prevents tissue breakdown. The patient does not need to do anything, and neither does the caregiver — it works through the night.
Bubble vs tubular: the choice that matters
Bubble (cell) mattresses
Made of many small circular air cells. Lower cost, lighter, adequate for patients at moderate risk who retain some ability to shift position. Typically lower weight capacity.
Tubular mattresses
Larger cylindrical air tubes running across the bed. Better pressure distribution, higher weight capacity, and generally the right choice for fully immobile patients, heavier patients, or anyone who already has a pressure sore.
Some tubular systems also allow individual tubes to be deflated under a specific wound site. If a sore already exists, mention it when enquiring — it changes the recommendation.
Where water beds fit
Water beds distribute weight across a fluid surface rather than alternating pressure. They are quiet, need no pump running constantly, and some caregivers prefer them. They are heavier, harder to move, and the temperature of the water matters for patient comfort. For most home-care situations in Kerala, an alternating pressure air mattress is the more practical choice, but water beds remain a valid option in some cases.
Getting the setup right
- Size it to the bed platform. An air mattress that overhangs or leaves gaps at the side rails is both less effective and a safety risk.
- Place the pump correctly. Usually hung at the foot of the bed. Keep tubing unkinked and the pump’s air intake clear.
- Set the pressure for the patient’s weight. Most pumps have a dial. Set too hard, the mattress stops conforming to the body; too soft and the patient bottoms out onto the frame. Check by sliding a hand under the sacrum — you should feel roughly 2–3 cm of air.
- Use minimal bedding between patient and mattress. Thick layers of sheets and draw sheets defeat the pressure redistribution. One fitted sheet, pulled smooth.
- Keep the sheet wrinkle-free. A fold in a sheet under an immobile patient is itself a pressure point.
The mattress is not the whole answer
Pressure care is a routine, not a product. Alongside the mattress:
- Reposition per the nursing advice given for the patient
- Inspect skin daily at sacrum, heels, hips, elbows and shoulder blades — look for redness that does not fade when pressed
- Keep skin clean and dry. Moisture from incontinence or sweat dramatically raises risk
- Maintain nutrition and hydration. Protein and fluid intake affect skin integrity directly
- Protect heels — heel cushions or pillows under the calves to float the heels clear
Any area of redness that does not fade within a few minutes of pressure being relieved should be reported to the treating clinician promptly. Early stages are reversible; later ones are not easily.
What to ask when buying
- Bubble or tubular, and which is right for this patient’s risk level?
- Maximum patient weight?
- Cycle time, and is it adjustable?
- Pump noise level — it will run all night beside a sleeping household
- Warranty on the pump specifically
- Are replacement cells or tubes available if one is punctured?
